Children's High Intensity Care Coordinator

DUTIES AND RESPONSIBILITIES:

Completes a comprehensive home-based assessment of the client’s needs and strengths
utilizing the Child and Adolescent Needs and Strengths Assessment (CANS) in
accordance with DMH protocols and standards.

Use the CANS assessment to develop, monitor, and periodically revise the care plan to
address medical, social, educational, and care coordination services. The Care Plan
should include activities to ensure active participation of the client, identify
specifications to respond to needs of the child, and include safety and/or crisis plan
developed with the child and his/her family.

Convene and facilitate the care planning team to identify strengths and needs of the
child and to identify the services, supports, and interventions to assist the child in
improving functioning and achieving and maintaining stabilization.

Work directly with the child / family to help in obtaining needed services to address
identified needs and achieve goals specified in the case plan. Advocate on behalf of the
child to secure community resources.

Services are delivered through home visits, telephone contacts, agency visits, and/or
family contacts.
Be available for after hours emergency calls.
Maintain a HICC to client ratio of, no more than, 1:18.
Meets required productivity expectations.

EDUCATION AND CERTIFICATION:

Education: Bachelor’s degree in a human services field.
Certificate/Licensure: Current Alabama driver's license and eligibility for coverage under
Center liability insurance and maintain a good driving record.
