JOB DESCRIPTION

POSITION TITLE: Children’s High Intensity Care Coordinator (HICC)
WAGE CLASSIFICATION: Non-Exempt
SALARY GRADE: HICC 
DIVISION: Limestone and Morgan County
HOURS: Full Time - 40 Hours Weekly - As Assigned
SUPERVISOR: Program Director, CTAC
POSITIONS SUPERVISED: None

DUTIES AND RESPONSIBILITIES:
• Completes a comprehensive home-based assessment of the client’s needs and strengths
utilizing the Child and Adolescent Needs and Strengths Assessment (CANS) in
accordance with DMH protocols and standards.
• Use the CANS assessment to develop, monitor, and periodically revise the care plan to
address medical, social, educational, and care coordination services. The Care Plan
should include activities to ensure active participation of the client, identify
specifications to respond to needs of the child, and include safety and/or crisis plan
developed with the child and his/her family.
• Convene and facilitate the care planning team to identify strengths and needs of the
child and to identify the services, supports, and interventions to assist the child in
improving functioning and achieving and maintaining stabilization.
• Work directly with the child / family to help in obtaining needed services to address
identified needs and achieve goals specified in the case plan. Advocate on behalf of the
child to secure community resources.
• Services are delivered through home visits, telephone contacts, agency visits, and/or
family contacts.
• Be available for after hours emergency calls.
• Maintain a HICC to client ratio of, no more than, 1:18.
• Meets required productivity expectations.

CRITICAL TASKS: Client Care.

EDUCATION AND CERTIFICATION:
• Education: Bachelor’s degree in a human services field.
• Certificate/Licensure: Current Alabama driver's license and eligibility for coverage under
Center liability insurance and maintain a good driving record.

MINIMUM EXPECTATIONS:
• Ability to relate well to clients, a multi-disciplinary staff and the community, ability to
work with a team. Must demonstrate and continue to show merit.
• TB screening prior to initiation of duties. Annual screening shall occur only if there has
been known exposure or evidence of ongoing TB transmission.
• Must be able to work in a cooperative manner with co-workers, supervisors, and other
staff in the company.
• Performs other related duties as required. (Related duties as required are duties which
may not be specifically listed in the duties and responsibilities description, but are
within the general occupational series and responsibility level typically associated with
the employee’s class of work.)
• Must be able to follow company policies and procedures.
• Must work the days and hours necessary to perform all assigned responsibilities and
tasks. Must be available (especially during regular business hours or shifts) to
communicate with supervisors, clients, vendors and any other person or organization
with whom interaction is required to accomplish work and project goals
• Must be punctual and timely in meeting all requirements of performance, including, but
not limited to, attendance standards, work deadlines, and beginning and ending
assignments on time.

TRAINING:
• Completion of a DMH approved Intensive Care Coordinator Training Program.
• Professionally accepted management of aggressive/assaultive behavior and crisis
intervention techniques:
o The training must have been within the past 2 years, and the training must have
been conducted prior to working alone with consumers.
• Infection control: Staff shall be trained prior to working with consumers and annually
thereafter.
• Initial training on the following topics:
o Diagnostic categories.
o Classes of psychotropic medications.
o Recovery orientation.
o Interaction with consumers and families.
• Abuse and neglect and all state laws pertaining to abuse and neglect including reporting
required by the Department of Human Resources. Training is required prior to working
alone with consumers and on an annual basis.

ADDITIONAL PREFERRED/DESIRED QUALIFICATIONS:
• Two years’ experience in mental health field.
